How to Register a Death
In most straightforward cases, this should be done within 5 days of the
death.
The name and address of the Registrar should be on the envelope containing
the Medical Certificate or can be found within a local phone book under
'R' (for Registrar).
The Registrar will require the following:
- Date and place of birth
- Maiden name of a married woman
- 'Usual' address of deceased
- Date and place of death
- Occupation of deceased
- The envelope containing the Medical Certificate
- If possible the NHS Medical Card belonging to the deceased
The registrar will provide you with a Green Certificate which is to be
handed to the Funeral Director without delay. You will also be given a
Death Certificate. You should also request two or three extra copies of
the Death Certificate for which a small charge is made (cash or cheque
required).
